There are times when the fixed appliance approach to molar uprighting is either inappropriate or not possible. Sometimes patients do not have enough teeth to use as an anchor or the stabilizing teeth are periodontally involved. A removable design allows you to use the soft tissue, teeth, and the appliance to form an anchor unit. A temporary partial can be designed which incorporates a spring or an expansion screw to accomplish the desired tooth movement. Retention and anchorage are accomplished by using two to three clasps and by having excellent tissue adaptation. This design is especially useful on adult patients with 12-year molars tipped mesially. Although not shown in this example, a labial bow is often used to prevent flaring of the anterior teeth. Note - When using removable appliances that have an expansion screw, have your patient activate the appliance one turn per week. This is equivalent to 1mm of movement per month.
